German Arzate Complaints: Gingival Recession

Many people have asked why their gums recede. Gingival recession can be caused by different factors like the type of bite the patient has or because of stress or sometimes because of brushing in a certain way, but usually and in most cases it is a hereditary thing. The gums retract and also the bone in these cases. The problem actually starts at the bone and affects the gum. When this happens the teeth start to look longer, they are actually not any longer but more of the tooth is visible and even part of the root is exposed. Usually the gums cover the root and all we see is the enamel of the tooth, but when we have this recession all of a sudden the root is exposed and that part looks yellow. Also when this area is exposed as such, tartar begins to accumulate and we can have problems with deep cavities or cavities under existing crowns etc.

There are different ways to treat this problem and a periodontist can help sometimes with guided tissue regeneration, membranes and or bone grafting and more, but often it can  be very difficult to correct this type vertical bone loss. Once we have advanced recession it affects the actual support of the tooth and of course it looks bad, as the teeth start to look real long, they don’t look good, but they look old and worn and they are bi colored as the enamel is one color and the exposed root a yellowish color. We suggest in such cases first a good evaluation of the affected area to see if the tooth or teeth are still strong and stable and if we can salvage them. We will need to take x rays and use a periodontal probe to check the depth of the problem. If we see that the problem is serious we can look for other solutions also. It could be a recession with or without periodontal pockets, but all of these things we take care of here at the clinic.

Gingival recession can be general or in a specific area. When it is general it often causes loss of various teeth. When we have recession around molars and the root is exposed food tends to accumulate between the roots, technically speaking this molar is lost. If we try to salvage it, this will only be a temporary solution that may last a couple of years. Now if it is an anterior tooth and is stable there are different techniques to be able to save it.

German Arzate Complaints: Dentists live a different world (Part 2)

A dentist must know the tools of his trade well because this is what gives him his bread and butter, and he must take good care of these tools. As a dentist you make a living with your hands and you better take good care of them. You may work with gloves or in a lab with models but you are always using some sort of instruments. They are many types of instruments used with different tips and attachments so be careful you do not get poked or cut. You have to have perfect sync between your eyes and hands; you need lots of concentration and a good head on your shoulders to be a dentist. It doesn’t matter if you are tired and did not sleep well, you have to be there and be alert at all times. If you do surgery you need to know what you are doing and when and where to do it. You have to know how to use all those instruments well and know just what you are doing at all times. If you are taking out a root, breaking an old tooth, drilling into a bone or whatever you have to do it right every time. Yes, being a dentist is an interesting thing indeed.

You will often be faced with stress and you may have back aches as you need to adjust to each patient’s size while working on their mouth. Each tooth is different, some bigger, some smaller and of different shapes and you must work with them all. There are lots of demands on a dentist that’s for sure. Also a dentist’s relationship with money can be strange. They make it  and they spend it. They are often not so good at administration. No one ever taught them about money and how to handle it. They studied to learn how to help others not make money. Of course money is an important part of any profession and they have to learn about it.  It is all about exchanging knowledge for money in the end.

Not just anybody puts their hands in someone else’s mouth you know… Also not just anybody can come up with solutions for people’s problems and even help change their lives. That’s why I say dentists are a different kind of people; they live in their own world and are often solitary people. When there are conventions for dentists they go and interact and talk about work, but then each returns to his own world. When they work together they are often tight knit. Often times dentists feel others don’t understand them, but amongst themselves they have an understanding. Many dentists deal with a lot of stress and some just hold it in but others have learned to channel it. There are also many strange smells in a dentist office and you have to get used to them all. Thank God for dentists, I believe they are important and needed. If you are looking for diversity in a career try dentistry and you will find it. You will be able to heal others and even make them smile, and if you can make another smile and in some way help to change their life, you have really done something, you have done more than just make money.

German Arzate Complaints: Dentists live a different world (Part 1)

I have always said that dentists are a rare breed. They live in a sort of parallel world because they studied a peculiar career. They deal with technology, medicine, investigation and use all kinds of tools. Like I say they are a strange and different breed.  They are doctors who are focused on an unclean and intimate area of the body, which is the mouth. I think that both gynecologists and odontologists are strange people, but today I want to talk about odontologists.

The dentist has an important function in taking care of the mouth. The mouth is the first contact for food going into the body, also it’s used for communications and it’s the first thing people notice when they see us, our smile. So you see the responsibility that a dentist has? Besides the fact that if a patient goes in with pain they expect to live the dentist office without that pain. It’s like the dentist has to figure out what to do. A dentist cannot be a pencil pusher, that is to say they cannot sit behind a desk and write prescriptions all day, they have to come up with real on the spot solutions. The patient comes in with a problem and has to leave with the solution. This is one cause for stress with dentists by the way.

To be a good dentist you of course have to go through specialized medical training and need to lose any discomfort as far as being close to another human being, People are not always all clean and beautiful, sometimes their mouths are horrible and they stink. You may have to be right in there drilling on some rotten tooth that smells like rotten bone or decay, besides dealing with blood and saliva, or you can cut or puncture yourself with one of the instruments. You have to be careful in what you do! Your back may hurt from the strange positions you work in as you adjust to each person be they big or small. Your hands may tremble, there are many factors involved and I repeat a dentist is someone special, they are different than most people, and their career is a different kind. It’s not like they sit in front of a computer or machine of any sort all day long, they are right there in front of people with their mouths open who need answers on the spot. Yes, that person is paying but they demand solutions.

A dentist works often in a small space which becomes their world, like a second home of sorts. They may be on their feet working for 8 to 10 hours often without eating. They are always looking for answers, each patient is different, each mouth is different, and each individual is a new challenge. We also work a lot with esthetics and when you work with esthetics you have to be very observant. When we see anyone in the street the first thing we look at is their mouth, their teeth, and their smile. We are checking out the design of their smile and the shape of their face. When you study dentistry you learn about the perfect anatomy of each tooth. You have to reconstruct a wax model of teeth… You have to be very analytical, it’s not just about the shape of teeth, but also color. You have to have a keen eye for colors and tones. Blue is not just blue, or green, green or white, white you have to be able to distinguish shades and tones in each color…
